摘要:
目的:探讨乌司他丁对体外循环(CPB)心脏手术患者围手术期心脏炎症反应状态的影响作用。方法:选取CPB心脏手术患者70例,采用SPSS16.0软件生成随机数字分为试验组(乌司他丁)和对照组(等量生理盐水)各35例,对比两组患者麻醉后切皮前(T1)、CPB 30 min(T2)、术毕(T3)、术后24 h(T4)血浆炎症反应因子及心肌酶学指标。结果:T1时刻,试验组和对照组的血浆IL-6、IL-8、TNF-α、外周血白细胞(WBC)、单核细胞测定值差异均无统计学意义(P >0.05);在T2、T3、T4时刻,试验组患者的血浆IL-6、IL-8、TNF-α、外周血WBC、单核细胞测定值均低于对照组(P <0.05),两组患者的血浆IL-6、IL-8、TNF-α、外周血WBC、单核细胞测定值均高于T1时刻(P< 0.05);T1时刻,试验组和对照组的血清CK-MB、cTnI水平差异均无统计学意义(P >0.05);在T2、T3、T4时刻,试验组患者的血清CK-MB、cTnI水平均低于对照组(P <0.05),两组患者的血清CK-MB、cTnI水平均高于T1时刻(P <0.05)。结论:乌司他丁可显著减轻CPB心脏手术患者围手术期炎症反应水平,对心肌具有一定的保护作用。
Abstract:
Objective:To investigate the effect of ulinastatin on inflammatory reaction of cardiopulmonary bypass (CPB) heart of perioperative cardiac surgery patients . Methods: CPB patients who underwent cardiac surgery in 70 cases were selected, SPSS16.0 software was used to generate the random number and experimental group (ulinastatin) 35 cases and control group (normal saline) 35 cases were dividied, two groups of patients with anesthesia before skin incision (T1), CPB 30 min (T2) and the end of operation (T3), 24 h after operation (T4) with inflammatory factors and myocardial enzymes were compared. Results:At T1,the difference of plasma IL-6, IL-8, TNF- alpha, peripheral WBC and mononuclear cells in the experimental group and the control group was not statistically significant (P >0.05); at T2, T3, T4, plasma IL-6, IL-8, TNF- alpha, peripheral WBC and monocytes in the test group were lower than the control group (P <0.05), and plasma IL-6, IL-8, TNF- alpha, peripheral WBC and monocytes in the two groups were higher than the T1 time (P <0.05); T1 moment, the differences of serum CK-MB and cTnI levels in experimental group and control group showed no statistical significance (P >0.05); at T2, T3, T4, serum CK-MB, cTnI levels in the test group were lower than the control group (P <0.05 ), and serum CK-MB, cTnI levels in two groups were higher than those at T1 times (P<0.05).ConclusionUlinastatin could significantly reduce the perioperative inflammatory response in patients undergoing CPB cardiac surgery, and has a protective effect on myocardium.
