 目的：分析自身免疫性肝炎（AIH）的临床特征。方法：回顾96例AIH患者的临床资料，分析其临床特征。结果：96例AIH患者平均发病年龄为（57.85±14.54）岁，男女比例为1：8.6。有身高、体质量资料的62例患者的平均体质量指数为（23.49±3.05）kg/m2。患者可无症状（36.46%）或伴有非特异性症状，存在不同程度的肝功能异常，以肝炎指标升高为主，65例（67.71%）免疫球蛋白G升高，81例（84.38%）抗核抗体阳性。所有患者中，经影像学和（或）肝组织活检证实37例（38.54%）存在肝硬化。72例进行了肝组织活检的患者中出现汇管区浆细胞浸润、界面性肝炎、肝细胞玫瑰花结改变和肝纤维化或肝硬化者分别为54例（75.00%）、51例（70.83%）、19例（26.39%）和23例（31.94%）。合并肝外自身免疫性疾病39例（40.63%）。结论：AIH好发于中年女性，无特异性临床症状，以血清肝炎指标和免疫球蛋白G升高为主，多存在自身抗体，肝组织学示肝细胞损害，常伴有肝外自身免疫性疾病。
Abstract:
Objective: To investigate the clinical characteristics of autoimmune hepatitis (AIH). Methods: The clinical data of 96 patients with AIH were retrospectively analyzed. Results: The average age of the 96 patients with AIH was (57.85±14.54) years old and the male to female ratio was 1:8.6. Height and weight data were acquired in 62 patients whose average body weight indices were between (23.49±3.05) kg/m2. All the patients were either asymptomatic (36.46%) or with nonspecific symptoms. The abnormalities of liver function varied mainly indicated by hepatic profile. Sixty-five patients(67.71%) presented elevated immunoglobulin G (IgG) and 81 cases (84.38%) were positive for antinuclear antibodies (ANA). Cirrhosis was diagnosed in thirty seven patients (38.54%) by imaging or liver biopsy. Portal plasma cell infiltration, interface hepatitis, rosettes of hepatocytes and liver fibrosis or cirrhosis were observed in 54 (75.00%), 51 (70.83%), 19 (26.39%) and 23 (31.94%) patients respectively among 72 patients receiving liver biopsy. Thirty-nine cases(40.63%) of all patients were complicated with extrahepatic autoimmune diseases. Conclusion: AIH affects middle-aged women more frequently with no specific symptoms, and is characterized by hepatic profile, elevated IgG and presence of autoantibodies in serology, hepatocellular injury in histology and other autoimmune diseases.
