摘要:
目的:研究心房颤动(AF)患者血浆血管性血友病因子(vWF)因子及D-二聚体水平在血栓形成及临床预后中的价值。方法:选取具有完整资料确诊AF患者61例,无AF史30例做为对照组,根据食道超声(TEE)及有无血栓史将AF患者分为血栓组(n=13)及非血栓组(n=48),测定D-二聚体及vWF含量,并评价血栓组行抗凝治疗后3个月的血栓情况,将其分为血栓溶解组(n=9)及血栓未溶解组(n=4),复测各组的两因子水平,进行分析评价。结果:与对照组比较,血栓组与非血栓组vWF水平及D-二聚体水平均高于窦性心律组,P<0.05。血栓组vWF水平及D-二聚体水平高非血栓组,P<0.05。血栓组经治疗后血栓溶解组两因子水平较治疗前明显下降(P<0.05),而血栓未溶解组两因子水平也较治疗前下降,但无统计学意义(P>0.05).且房颤血栓组血浆D-二聚体及vWF水平呈正相关性(r=0.952, P<0.05)。结论:vWF因子及D-二聚体水平可用于评估AF患者血栓形成风险的高低,同时可以预测合并血栓的AF患者的临床预后。
Abstract:
?Objective: To study the value of von Willebrand factor(vWF)and plasma D-dimer in patients with non-valvular atrial fibrillation (AF) for thrombosis and prognosis . Methods: Sixty-one patients with AF and thirty patients with non-AF were enrolled into the study, patients with AF according to transesophageal echocardiography (TEE) and history of thrombosis were divided into two groups, thrombus group(n=13) and non-thrombus group (n=48), then the plasma concentration of vWF and D-dimer were measured. Furthermore, thrombus group was divided into thrombolysis group and non-thrombolysis group through anticoagulant treatment, after anlysing and evaluating the results. Results: Compared with the control group, the palsma levels of vWF and D-dimer were all significantly higher (P<0.05) in patients in thrombus group and non-thrombus group. Compared with thrombus group, the levels of vWF and D-dimer were higher in patients of non-thrombus group. The levels of the two factors were significantly lower in thrombolysis group compared with prior treatment, but in non-thrombolysis group they showed no statistical significance before and after the treatment. In addition, vWF was positively correlated with D-dimer(r=0.952, P<0.05). Conclusion: The level of vWF and D-dimer could estimate the risk of thrombus formation in AF patients, as well as make good prognosis.
