摘要:
胃癌确诊时大多已出现转移，且以淋巴结转移为主。主要治疗手段是以胃癌根治术为主的综合治疗，随着近年来淋巴结清扫范围的扩大，胃癌术后乳糜漏发生率也随之升高。乳糜漏在D2以上淋 巴结清扫的发生率相对较高，与淋巴结清扫范围、肿瘤位置及分期等因素密切相关，可使大量水分、电解质及蛋白质丢失, 导致营养不良及电解质紊乱等一系列临床问题。乳糜漏应以预防为主，术时发 现淋巴管损伤应及时结扎或修补。术后乳糜漏应以保守治疗为基础，保守治疗效果不明显时根据患者具体情况决定是否行介入或手术治疗。
