摘要:
目的：分析神经外科重症监护室（NICU）中脑外伤感染泛耐药鲍曼不动杆菌患者的临床特征，为优化该难治性感染的防治策略提供依据。方法：回顾性分析12例脑外伤（TBI）感染泛耐药鲍曼不动杆菌患者流行病学、伤型、入院时伤情、应用抗生素时间和种类、感染部位、细菌种类、血常规、营养、气道管理情况等参数特点与患者结局的相关关系。结果：鲍曼不动杆菌的脑外伤患者以男性居多，伤情较重的TBI居多，第1次应用抗生素时间平均为入院后（19.2±16.6）d；持续使用抗生素时间（40.4±25.9）d；使用抗生素种类平均（3.8±1.2）种；感染部位均为下呼吸道；感染泛耐药鲍曼不动杆菌前培养阳性最多的细菌为肺炎克雷伯杆菌与耐甲氧西林金黄色葡萄球菌，感染者气管切开、气管插管和呼吸机辅助呼吸者比例多，确诊后9例应用美罗培南+头孢哌酮舒巴坦钠治疗；不同伤情和不同伤后平均血红蛋白水平的预后不同。结论：TBI感染泛耐药鲍曼不动杆菌的易感因素为伤情较重、有创气道管理、长期使用抗生素以及频繁更换抗生素等。在没有太多药物方案选择条件下，维持患者基本营养可能是促使其转归偏好的主要因素。
