摘要:
目的:探讨患者血清中的白细胞介素-6(IL-6)、降钙素原(PCT)及C反应蛋白(CRP)对发生腹腔感染(IAI)的诊断意义。方法:选取2024年3月—11月河北大学附属医院普通外科收治的IAI患者57例为病例组,同期选取收治的非感染性疾病患者41例为对照组。比较两组患者IL-6、PCT和CRP水平,评估它们的诊断效能。结果:病例组血清IL-6、PCT和CRP水平明显高于对照组(Z=-6.259、-6.080、-3.897,均P<0.05)。Logistic回归分析表明,血清IL-6、PCT和CRP升高均是患者发生IAI的独立危险因素(OR=1.040、4.244、1.014,95%CI:1.018~1.062、1.073~16.780、1.002~1.026,均P<0.05)。受试者工作特征(ROC)曲线的结果分析显示,IL-6、PCT、CRP及联合检测诊断腹腔感染的ROC曲线下面积分别为0.872、0.861、0.731、0.914,差异具有统计学意义(敏感度分别为98%、74%、54%、90%,特异性分别为66%、85%、80%、85%,均P<0.05)。结论:IL-6、PCT及CRP联合检测可提高患者发生腹腔感染的诊断效能。
Abstract:
Objective: To discuss the diagnostic significance of interleukin-6 (IL-6), procalcitonin (PCT) and C-reactive protein (CRP) in the serum of patients with intra-abdominal infection (IAI). Methods: A total of 57 patients with IAI admitted to the Department of General Surgery, Affiliated Hospital of Hebei University from March to November in 2024 were selected as the case group, and 41 patients with non-infectious diseases were selected as the control group during the same period. The expression of IL-6, PCT and CRP were compared between the two groups of patients to evaluate their diagnostic efficacy. Results: The serum levels of IL-6, PCT and CRP in case group were significantly higher than those in control group (Z=-6.259, -6.080, -3.897, all P<0.05). Logistic regression analysis showed that the increase of serum IL-6, PCT and CRP were independent risk factors for IAI in patients (OR=1.040, 4.244, 1.014, 95%CI:1.018-1.062, 1.073-16.780, 1.002-1.026, all P<0.05). The analysis of the receiver operating characteristic (ROC) curve results showed that the areas under the ROC curves for IL-6, PCT, CRP, and the combined detection for diagnosing intra-abdominal infections were 0.872, 0.861, 0.731, and 0.914, respectively, with statistically significant differences (sensitivity was 98%, 74%, 54%, and 90%, while specificity was 66%, 85%, 80%, and 85%, all P<0.05). Conclusion: Combined detection of IL-6, PCT and CRP can improve the diagnostic efficacy for intra-abdominal infections.
