目的:探讨儿童肝移植术后巨细胞病毒(CMV)感染患儿外周血单个核细胞(PBMCs)中γ干扰素(IFN-γ)、白细胞介素-10(IL-10)、转化生长因子β1(TGF-β1) mRNA表达水平的变化。方法:收集天津市第一中心医院器官移植中心儿童器官移植科行儿童肝移植患儿40例,根据术后是否发生CMV感染分为CMV组(20例)和未感染组(20例)。分离外周血单个核细胞(PBMCs),应用TaqMan实时荧光聚合酶链反应(RT-PCR)测定IFN-γ、IL-10及TGF-β1 mRNA表达水平。结果:与未感染组比,CMV组IFN-γ mRNA表达水平显著降低(t=3.759,P<0.01);TGF-β1 mRNA表达水平显著上升(t=-2.478,P<0.05);IFN-γ/IL-10比值显著降低(Z=-3.260,P<0.01);IFN-γ mRNA表达水平与TGF-β1 mRNA表达水平呈负相关(r=-0.555,P<0.05);IFN-γ及IFN-γ/IL-10比值诊断肝移植患儿CMV感染的曲线下面积(AUC)分别为0.791(95% CI:0.649~0.933)、0.801(95% CI:0.658~0.945)。抗病毒治疗后CMV组IFN-γ mRNA表达水平较治疗前显著上升(t=2.407,P<0.05)。结论:肝移植术后CMV感染患儿PBMCs中IFN-γ mRNA低表达可能与CMV感染的发病机制相关。
Abstract:
Objective: To investigate the changes of interferon gamma (IFN-γ), interleukin 10 (IL-10), and transforming growth factor beta 1 (TGF-β1) mRNA expression in peripheral blood mononuclear cells (PBMCs) in pediatric liver transplantation with cytomegalovirus (CMV) infection. Methods: A total of 40 pediatric patients who underwent pediatric liver transplantation were enrolled in the Department of Pediatric Organ Transplantation, Organ Transplantation Center, Tianjin First Central Hospital. Patients were divided into the CMV group (n=20 cases) and the non-infected group (n=20 cases) according to whether CMV infection occurred after surgery. Peripheral blood mononuclear cells (PBMCs) were isolated, the expression of IFN-γ, IL-10 and TGF-β1 mRNA in PBMCs were measured by TaqMan real-time reverse transcriptase-polymerase chain reaction (RT-PCR). Results: Compared with the non-infected group, the expression of IFN-γ mRNA was significantly decreased in the CMV group (t=3.759, P<0.01), the expression of TGF-β1 mRNA was significantly increased in the CMV group (t=-2.478, P<0.05), and IFN-γ/IL-10 ratio was significantly decreased in the CMV group (Z=-3.260, P<0.01). In the CMV group, expression of IFN-γ mRNA was negatively correlated with expression of TGF-β1 mRNA (r=-0.555, P<0.05). The area under the curve (AUC) for IFN-γ and IFN-γ/IL-10 ratio in diagnosing CMV infection in children after liver transplantation was 0.791 (95%CI:0.649-0.933) and 0.801 (95%CI:0.658-0.945), respectively. After antiviral treatment, the expression of IFN-γ mRNA in the CMV group significantly increased compared with that before treatment (t=2.407, P<0.05). Conclusion: The low expression of IFN-γ mRNA may be related to the pathogenesis of CMV infection in PBMCs of children with CMV infection after liver transplantation.
