目的:分析冠心病患者经皮冠状动脉介入治疗(PCI)术后,血清血管细胞黏附分子-1(VCAM-1)、脂蛋白a[Lp(a)]、N-末端B型利钠肽前体(NT-proBNP)与预后的关系。方法:回顾性分析2021年9月至2023年9月在安阳地区医院心内科接受PCI手术治疗的 78 例冠心病患者的临床资料,依据术后随访 6 个月中患者主要不良心血管事件(MACE)的发生情况进行分组:MACE组(n=33)、无MACE组(n=45)。比较两组病例的一般资料、血清VCAM1、Lp(a)和NT-proBNP水平。采用多因素Logistic回归分析冠心病患者PCI术后预后不良的影响因素。通过受试者工作特征(ROC)曲线分析血清VCAM1、Lp(a)和NT-proBNP水平,预测其在冠心病患者PCI术后预后不良中的价值。结果:与无MACE组相比,MACE组血清VCAM1(t=5.488,P<0.001)、Lp(a)(t=5.352,P<0.001)、NT-proBNP(t=3.934,P<0.001)明显升高;经多因素Logistic回归分析证实:VCAM1(OR=1.542,95%CI:0.127~2.110,P=0.007)、Lp(a)(OR=3.198,95%CI:1.243~8.228,P=0.016)、NT-proBNP(OR=2.483,95%CI:1.305~4.724,P<0.001)水平升高是冠心病患者PCI术后预后不良的影响因素;经ROC曲线分析证实VCAM1[曲线下面积(AUC)=0.839,95%CI:0.745~0.925,P<0.001]、Lp(a)(AUC=0.773,95%CI:0.674~0.871,P<0.001)、NT-proBNP(AUC=0.790,95%CI:0.691~0.889,P<0.001)水平可用于预测冠心病患者PCI术后的预后。结论:血清VCAM1、Lp(a)、NT-proBNP均为冠心病患者PCI术后预后不良的影响因素,其水平的升高将增加患者预后不良发生风险。
Abstract:
Objective: To analyze the relationship between serum vascular cell adhesion molecule-1 (VCAM-1), lipoprotein a [Lp(a)], N-terminal B-type natriuretic peptide (NT-proBNP) and prognosis in patients with coronary heart disease after percutaneous coronary intervention (PCI). Methods: The clinical data of 78 patients with coronary heart disease who underwent PCI in the Depar-tment of Cardiology of Anyang Regional Hospital from September 2021 to September 2023 were retrospectively analyzed, and the patients were divided into major adverse cardiovascular events (MACE) group (n=33) and non-MACE group (n=45) according to the occurrence of MACE within 6 months of postoperative follow-up. General data, serum VCAM1, Lp(a) and NT-proBNP levels were compared between the two groups. Multivariate Logistic regression was used to analyze the influencing factors of poor prognosis in patients with coronary heart disease after PCI. The value of serum VCAM1, Lp(a) and NT-proBNP levels in predicting poor pro- gnosis of patients with coronary heart disease after PCI was analyzed by receiver operating characteristic (ROC) curve. Results: The serum VCAM1 (t=5.488, P<0.001), Lp (a) (t=5.352, P<0.001), and NT proBNP (t=3.934, P<0.001) levels in the MACE group were higher than those in the non-MACE group. Multivariate Logistic regression analysis confirmed that VCAM1 (OR=1.542, 95%CI:0.127~2.110, P=0.007),Lp(a)(OR=3.198,95%CI:1.243~8.228,P=0.016),NT-proBNP(OR=2.483,95%CI:1.305-4.724, P<0.001) levels were the influential factors for poor prognosis of patients with coronary heart disease after PCI. ROC curve analysis confirmed that VCAM1 [area under the curve (AUC) =0.839, 95%CI: 0.745-0.925, P<0.001], Lp(a) (AUC=0.773, 95%CI: 0.674-0.871, P<0.001), NT-proBNP (AUC =0.790, 95%CI: 0.691-0.889, P<0.001) levels could be used to predict the prognosis of patients with coronary heart disease after PCI. Conclusion: Serum VCAM1, Lp(a) and NT-proBNP are all influential factors for poor prognosis in patients with coronary heart disease after PCI, and the increase of them will increase the risk of poor prognosis in patients.
