摘要:
目的:通过心率变异性分析(HRV)从自主神经系统角度评价右美托咪定(DEX)术前抗焦虑作用。方法: 2017年6月至2020年10月天津医科大学口腔医院择期手术患者80例,年龄18~60岁,将患者按照简单随机化分组法分为DEX组和生理盐水组,每组40例。术前45 min DEX组患者进行经鼻腔喷雾DEX 0.75 μg/kg,生理盐水组用同样方式给予等容量生理盐水。记录患者术前一日(T0)状态焦虑问卷(SAI)分值,入室(T1)时脑电双频指数(BIS)与Ramsay镇静评分,T0和T1时收缩压、舒张压、心率和HRV分析指标。通过比较组内及组间各指标结果评判本次试验的临床效果。结果: 生理盐水组:T1与T0相比,心率显著升高(t= -2.372,P<0.05);全部相邻RR间期差值的均方根(RMSSD)、高频功率的对数值(logHF)、散点图短轴(SD1)、心率减速力(DC)显著降低(t=3.908、2.043、3.471、3.550,均P<0.05),全部窦性RR间期的标准差(SDNN)、低频与高频比值(LF/HF)、散点图长轴(SD2)显著升高(t=-2.435、-5.621、-2.885,均P<0.05)。DEX组:与T0相比,T1的心率显著降低(t=3.142,P<0.05)。T1时DEX组与生理盐水组相比,BIS值显著降低而Ramsay镇静评分显著升高(t=3.441、-2.078,均P<0.05);RMSSD、logHF、SD1、DC显著升高(t=-2.920、-5.135、-6.351、-6.021,均P<0.05),而SDNN、LF/HF、SD2显著降低(t=2.331、7.868、3.900,均P<0.05)。结论: 右美托咪定术前鼻喷具有较好的抗焦虑作用,HRV分析作为一种客观、量化的方法,可对患者焦虑的评价进行有益的补充。
Abstract:
Objective: To evaluate the preoperative anxiety effect of dexmedetomidine(DEX) from the perspective of autonomic nervous system by heart rate variability(HRV) analysis. Methods: From June 2017 to October 2020,eighty patients scheduled for surgery in the Stomatological Hospital of Tianjin Medical University,aged 18-60 years,were randomly divided into DEX and saline groups,40 cases in each group. About 45 minutes before the operation,DEX group received DEX 0.75 μg/kg via nasal spray,and normal saline group received equal volume of normal saline in the same way. The score of state anxiety inventory(SAI) the day before operation(T0),Bispectral index (BIS) and Ramsay Sedation score of entering room(T1),systolic blood pressure,diastolic blood pressure,heart rate and HRV analysis indicators at T0 and T1 were recorded. The clinical effects of this experiment were evaluated by comparing the results of intra-and inter-group indexes. Results: In normal saline group,heart rate was significantly higher at T1 than that at T0 (t=-2.372,P < 0.05).The root of the averaged squares of differences between neighboring RR intervals (RMSSD),the logarithm of high frequency power (logHF),the standard deviation of the short-term variability in Poincare plot (SD1) and deceleration capacity of heart rate(DC) decreased significantly(t=3.908, 2.043,3.471,3.550,all P<0.05),while the standard deviation of all RR intervals(SDNN),ratio of low frequency power to high frequency power (LF/HF) and the standard deviation of the long-term variability in Poincare plot(SD2) increased significantly(t=-2.435,-5.621,
