摘要:
目的:探讨影响经改良胃管微创注入肺表面活性物质(LISA)技术治疗新生儿呼吸窘迫综合征疗效的相关影响因素。方法:纳入2015年6月—2020年6月住院治疗的80例新生儿呼吸窘迫综合征(NRDS)患儿作为研究对象,进行经胃管LISA技术治疗。按照是否需要再插管通气,分为有效组68例,无效组12例。收集患儿的一般资料、临床资料、治疗过程等数据。对有意义的指标进行多因素Logistic分析。结果:无效组早产儿、双胎/多胎妊娠、剖宫产、胎儿宫内窘迫的比例明显高于有效组(?字2=8.162、7.843、7.517、9.461,均P<0.05);无效组糖皮质激素比例明显低于有效组(?字2=9.043,P<0.05);无效组胎龄、出生体重、1 min Apgar评分、5 min Apgar评分、生后72 h红细胞比容、pH值、O2分压明显低于有效组(?字2=2.639、4.036、5.020、4.130、2.160、2.156、4.326,均P<0.05);无效组胸部X线(重度)、CO2分压明显高于有效组(?字2=11.060、11.060,均P<0.05)。Logistic多因素分析结果显示,胎龄(OR=2.063,P=0.003)、糖皮质激素(OR=0.846,P=0.001)、1 min Apgar评分(OR=0.460,P=0.015)、胸部X线等级(OR=0.552,P=0.006)均是NRDS的影响因素。结论:糖皮质激素为影响LISA技术治疗疗效的保护因子,而胎龄、1 min APgar评分、胸部X线等级是危险因子。
Abstract:
Objective: To investigate the related factors that affect the efficacy of the modified gastric tube minimally invasive injection of pulmonary surfactant(LISA) technology in the treatment of neonatal respiratory distress syndrome. Methods: A total of 80 children with neonatal respiratory distress syndrome(NRDS) hospitalized from June 2015 to June 2020 were enrolled as the research subjects,and were treated with transgastric tube LISA technology. According to the need for reintubation,68 cases were divided into effective group and 12 cases were ineffective group. General data,clinical data,treatment process and other data of children were collected. Multivariate Logistic analysis was used to analyze the significant indexes. Results: The proportion of premature babies,twin/multiple pregnancy,cesarean section,and intrauterine distress in ineffective group were significantly higher than those in effective group(?字2=8.162,7.840,7.517,9.461,all P<0.05); the proportion of glucocorticoids in ineffective group was significantly lower than effective group(?字2=9.043,P<0.05);in ineffective group,gestational age,birth weight,1 min Apgar score,5 min Apgar score,72 h postnatal hematocrit,pH,and O2 partial pressure were significantly lower than those in effective group( ?字2=2.639,4.036,5.020,4.130,2.160,2.156,
