摘要:
目的:探讨血小板-淋巴细胞比值(PLR)与冠状动脉病变复杂性的相关性及临床意义。方法:纳入天津市胸科医院就诊的急性冠脉综合征(ACS)患者,检测血常规和生化指标,行冠脉造影后计算SYNTAX积分,根据SYNTAX积分分为:SYNTAX低值组(SYNTAX<23)和SYNTAX中高值组(SYNTAX≥23)。结果:与SYNTAX低值组相比,SYNTAX中高值组血糖、肌酐、尿酸、低密度脂蛋白、白细胞计数、血小板计数和PLR均升高,而淋巴细胞计数减低,差异具有统计学意义(P<0.05)。应用多因素Logistic回归分析显示PLR、年龄、糖尿病、既往心肌梗塞史、白细胞、LDL与SYNTAX积分显著相关(P<0.05)。结论:入院时PLR与急性冠脉综合征患者冠脉复杂性显著相关,提示PLR增加是急性冠脉综合征患者SYNTAX积分较高的独立预测因素。
Abstract:
Objective: The aim of this study was to investigate the correlation between Platelet-to-Lymphocyte ratio(PLR) and the complexity of coronary artery disease assessed by the SYNTAX score(SXscore) in patients with acute coronary syndrome(ACS). Methods: A total of 462 patients with ACS who underwent CAG were included in the study from January 2010 to June 2010. The SXscore was determined from baseline CAG. The patients were divided into 2 groups, those with low SXscores(<23) and those with intermediate to high SXscores(≥23) . History data, clinical and laboratory results were retrospectively analyzed. Results: PLRs were significantly higher in patients with intermediate to high SXscores compared with those with low SXscores. In multivariate analysis, the independent predictors of intermediate to high SXscore were PLR together with the age, diabetes mellitus, OMI, white blood cell and LDL . Conclusion: The PLR is significantly associated with the severity and complexity of coronary atherosclerosis in patients with ACS. Increased PLR is an independent predictor of higher SXscore in patients with ACS.
