摘要:

目的: 提高临床医生对Ⅰ型塑型性支气管炎(PB)的认识。方法: 采用回顾性病例分析方法对73例PB病人的临床资料进行分析。结果: 73例病例中男52例,女21例,男:女比为2.5:1;年龄10月~12岁,平均年龄(6.1±2.8)岁。所有病例均有发热,其中68例(占93.1%)高热,热程平均(13.9±7.6)d;咳嗽亦为主要表现,干咳65例(占89.0%)。胸部影像学显示不同表现,72例(占98.6%)为肺实变/肺不张;33例(占45.2%)合并胸腔积液,5例(占6.8%)合并肺气肿,2例(占2.7%)合并液气胸。15例(占20.5%)患者有呼吸困难,其中9例(占12.3%)出现低氧血症,2例(占2.7%)应用无创机械通气治疗。病原学检查发现66例(占90.4%)肺炎支原体(MP)阳性,有43例(占58.9%)存在混合感染,7例(占9.6%)为其他病原体感染。所有患者外周血常规无特征性改变,65例患者(占89.0%)LDH升高,57例(占78.1%)ESR增快,56例(占76.7%)CRP升高,64例(占87.6%)IL-6升高,45例(占61.6%)D-二聚体升高。所有患儿均于入院后1~9 d(病程5~33 d)行纤维支气管镜检查,镜下发现病变部位支气管可见灰白色分泌物栓塞,栓塞物经病理诊断均为Ⅰ型PB。所有病例均以临床好转或治愈出院,平均住院(10.1±4.1)d。结论: MP感染是儿童Ⅰ型PB的主要病原体,持续高热及影像学进展的病人,应警惕PB发生,及早行纤维支气管镜治疗。
