摘要:
目的:探讨黄连素对精神分裂症患者精神病性症状、糖脂代谢的影响。方法:选取64例住院精神分裂症患者,口服单一抗精神病药利培酮,将其随机分为两组:研究组服用利培酮+黄连素(300 mg, 3次/d),对照组服用利培酮+安慰剂(300 mg, 3次/d), 治疗12周。比较治疗后外周血空腹血糖(FPG)、空腹胰岛素(Fins)、胆固醇(TC)、甘油三酯(TG)、载脂蛋白A1 (Apo-A1)及载脂蛋白B (Apo-B) 水平;并评估阳性及阴性症状量表(PANSS)、临床疗效总评量表(CGI)、不良反应量表(TESS)。结果:(1)治疗12周末,研究组在 PANSS 阳性症状得分[(10.93±2.23)分]与对照组[(12.24±2.40)分]相比,差异有统计学意义(t=2.204;P=0.031)。 (2)治疗12周末,研究组FPG[(4.42±0.33)mmol/L]、Fins[(7.07±3.10)pmol/L]与对照组相比FPG[(5.59±0.60)mmol/L]、Fins[(9.73±3.05)pmol/L],差异有统计学意义(t=-9.199,P=0.000; t=-3.401,P=0.001);研究组TC[(4.52±0.55)mmol/L]、TG[(1.34±0.62)mmol/L]、Apo-A1[(1.74±0.61)mmol/L]、Apo-B[(0.93±0.16)mmol/L]与对照组相比TC[(4.91±0.59)mmol/L]、TG[(1.86±0.83)mmol/L]、Apo-A1[(1.24±0.27)mmol/L]、Apo-B[(1.11±0.17)mmol/L],差异有统计学意义(t=-2.649,P=0.010;t=-2.573,P=0.008;t=4.245,P=0.000;t=-4.154,P=0.000)。(3) 比较治疗后两组发生血糖升高及体质量增加不良反应的例数,研究组[1例(3.20%),5例(6.50%)]明显低于对照组[8例(24.20%),14例(27.30%)],差异有统计学意义(χ2=5.842,P=0.016;χ2=5.295,P=0.021)。其余各不良反应的差异均无统计学意义(P>0.05)。结论:黄连素可有效拮抗利培酮引起的糖脂代谢异常,改善精神分裂症患者的阳性症状且安全性良好。
