摘要:
目的: 探讨2型糖尿病(T2DM)患者代谢综合征(MS)组分与血清谷氨酰转肽酶(γ-GGT)的相关性。方法:445例T2DM患者按MS组分数目分为A、B、C、D组,分别包含1、2、3、4个MS组分,按血清γ-GGT水平分为正常组与异常组,比较不同组间γ-GGT、血糖、血压、血脂、胰岛素及胰岛素抵抗指数(HOMA-IR)等指标。结果:(1) 随MS组分的增加,BMI、TG、SUA、γ-GGT水平逐渐升高,差异有统计学意义(P<0.05);C、D组SBP、DBP、TC、LDL-C、HDL-C、HOMA-IR明显高于A、B组,D组HDL-C高于C组,B组DBP、HOMA-IR高于A组,差异有统计学意义(P<0.05)。(2)γ-GGT异常组年龄轻、病程短,且BMI、DBP、TG、FIns、HOMA-IR、SUA、ALT、AST高于正常组(P<0.05)。(3)Pearson相关分析显示,γ-GGT与BMI、DBP、SUA、TG、FIns、HOMA-IR呈正相关(r=0.296、0.134、0.411、0.106、0.089、0.082,P<0.05),与年龄、病程呈负相关(r=-0.088、-0.188,P<0.05)。多元逐步线性回归分析显示,γ-GGT与BMI、SUA、TG、HOMA-IR呈正相关(B=0.016、0.002、0.081、0.078,P<0.05),与病程呈负相关(B=-0.011,P<0.05)。(4)Logistic回归分析示,BMI、TG、FIns、SUA是血清γ-GGT升高的独立危险因素。结论:T2DM患者MS组分与血清γ-GGT水平密切相关,BMI、TG、FIns、SUA是血清γ-GGT升高的独立危险因素。
Abstract:
Objective: To investigate the association of metabolism syndrome(MS)and its components with serum γ-glutamyl transpeptidase(γ-GGT) in patients with type 2 diabetes. Methods: Four hundred and forty five type 2 diabetic patients according to the MS group number were divided into A, B, C and D group, respectively with 1, 2, 3, 4 MS components. And then patients were divided into normal and abnormal groups according to the levels of serumγ-GGT. γ-GGT, blood glucose, blood pressure, blood lipid, insulin and insulin resistance index (HOMA-IR) and other indicators of different groups were compared. Results:(1) As the increase of MS component, the level of BMI,TG,SUA,γ-GGT increased gradually(P<0.05);Compared with group A,B, the levels of SBP,DBP,TC,LDL-C,HDL-C,HOMA-IR increased significantly in group C, D and the level of HDL-C in group D was higher than group C. The level of DBP, HOMA-IR in group B were higher than group A(P<0.05). (2) Abnormal γ-GGT group presented with young age and short duration, and compared with normal group, the levels of BMI, DBP, TG, FIns, HOMA-IR, (3 SUA,ALT,AST in the group increased significantly (P<0.05).) Pearson correlation analysis revealed γ-GGT had a positive was positively correlated with BMI, DBP, SUA, TG, FIns, HOMA-IR(r=0.296,0.134,0.411,0.106,0.089,0.082,P<0.05),and a negative correlation with age and duration(r=-0.088,-0.188,P<0.05). Multiple linear regression showed γ-GGT was positively correlated with BMI,SUA,TG, HOMA-IR(B=0.016,0.002,0.081,0.078,P<0.05),and a negative correlation with duration(B=-0.011,P<0.05).(4)Logistic regression showed BMI, TG, FIns, SUA were independent risk factors for elevated surem γ-GGT. Conclusion: In patients with type 2 diabetes, MS and its components are closely corelated with surem γ-GGT and BMI,TG,Fins and SUA are independent risk factors for elevated surem γ-GGT.
