摘要:
目的:探讨原发性高血压患者应用动态动脉硬化指数(AASI)对冠心病的预测价值。方法:入选高血压患者226例, 所有患者均行24 h动态血压(ABPM)监测并计算AASI,行冠状动脉造影检查并依据造影结果进行Gensini评分(CSS),按照评分结果分为3组:0~1分的非冠心病组(70例),2~31分的轻、中度冠脉病变组(104例),≥32分的重度冠脉病变组(52例),采用单因素方差分析及Logistic回归分析与冠脉相关的危险因素。结果:单因素分析结果显示,3组间AASI差异显著(P<0.05),组间两两比较在AASI水平上仍有统计学意义(P<0.05),即冠脉狭窄越严重,AASI越高;Logistic回归分析显示,调整年龄、24 h心率、血压杓型等影响因素后,AASI与冠心病的发生仍具有较好的相关性(OR=3.471,P<0.01)。结论:AASI对高血压患者出现冠脉病变及其严重程度有较好的预测价值。
Abstract:
Objective: To investigate the role of ambulatory arterial stiffness index (AASI) to predict coronary heart disease(CHD) in hypertensive patients. Methods: A total of 226 hypertensive inpatients were included in the study. AASI was obtained from individual 24-hour blood pressure readings. The subjects were divided into three groups according to coronary stenosis score(CSS) after coronary angiography to evaluate the severity of coronary artery lesions : non CHD(0~1 score)group, mild lesion(2~31 scores)group and severe lesion(≥32 scores)group. The differences between the three groups were compared. The correlation between AASI and CHD were analyzed with Logistic regression. Results: AASI was significantly higher in patients with soft and severe artery lesions of CHD than in non CHD(P<0.05) .With the CSS increasing, the patients had higher AASI . Correlation between AASI and CHD was significant by Logistic regression after adjusting for confounding factors(P<0.05). Conclusion: AASI is closely related to the CHD and the severity of coronary artery stenosis.
