摘要:
目的:探讨弥散张量成像(DTI)序列在脊髓型颈椎病(CSM)的疗效评价中的应用价值。方法:选取60例改良日本骨科协会颈脊髓功能评分(mJOA)在12分以上的CSM患者,对所有患者行中西医综合康复治疗,每日1次,治疗两周。所有患者治疗前后均进行MRI检查,以患者受累部位为节点,设置一个感兴趣区,DTI扫描后记录FA值和ADC值。治疗前后均对患者进行mJOA评分和颈椎功能障碍指数评分(NDI)评定。临床评分分别和FA值与ADC值做Pearson相关性分析。结果:治疗后,mJOA评分、FA值大于治疗前,NDI评分、ADC值小于治疗前,差异存在统计学意义(t=9.79、1.25、13.95、16.58,均P<0.05);FA值和mJOA评分及其治疗前后的差值均呈正相关(r=0.389、0.345,均P<0.01);ADC值与mJOA评分及其治疗前后的差值均呈负相关(r=-0.472、-0.526,均P<0.01),ADC值与NDI评分及其治疗前后的差值均呈正相关(r=0.446、0.345,均P<0.01);FA值和NDI评分及其治疗前后的差值均无相关性(均P>0.05)。结论:DTI序列在CSM的疗效评价上存在积极意义。
Abstract:
Objective:To explore the application value of diffusion tensor imaging(DTI) sequences in the efficacy evaluation of cervical spondylotic myelopathy(CSM). Methods:Sixty CSM patients with a modified Japanese Orthopedic Association cervical spinal cord function score(mJOA) of 12 or above were selected,and all patients underwent comprehensive rehabilitation treatment with both traditional Chinese and Western medicine,once a day,for two weeks. All patients underwent MRI examination before and after treatment,with the affected area of the patient as the node,and an area of interest was set. After DTI scanning,FA and ADC values were recorded. Both mJOA score and Cervical Dysfunction Index(NDI) score were performed on patients before and after treatment. Pearson correlation analysis was performed between clinical scores,FA values,and ADC values. Results:After treatment,the mJOA score and FA value were higher than those before treatment,while the NDI score and ADC value were lower than those before treatment,and the differences were statistically significant(t=9.79,1.25,13.95,16.58,all P<0.05).The FA value and mJOA score,as well as the difference before and after treatment,were positively correlated(r=0.389,0.345,both P<0.01).The ADC value was negatively correlated with the mJOA score and its difference before and after treatment(r=-0.472,-0.526,both P<0.01),and positively correlated with the NDI score and its difference before and after treatment(r=0.446,0.345,both P<0.01). There was no correlation between FA value,NDI score,and the difference before and after treatment(both P>0.05). Conclusion:DTI sequences have a positive significance in evaluating the efficacy of CSM.
