摘要:
目的：探讨交叉韧带囊肿的发病、治疗，并评估关节镜手术疗效。方法：采用关节镜下囊肿切除术治疗交叉韧带囊肿35例，其中男22例，女13例，年龄17～44岁，平均（30.4±7.1）岁。所有患者均为单侧发病，左膝15例，右膝20例。其中32例患者纳入随访研究，所有患者均以疼痛伴伸屈功能受限为主要症状，32例患者中，单纯伸直受限20例（62.5%），伸直屈曲均受限4例（12.5%），单纯屈曲受限8例（25%）。术后对症治疗并指导功能锻炼。术后随访并测量患肢伸屈受限角度，收集VAS评分，Lysh?觟lm评分及查体情况来评估手术疗效，转科查体来评估关节稳定性。结果：32例患者获得随访，平均随访时间（38.5±9.4）个月（17～51个月）。膝关节屈伸受限角度由术前（11.62± 3.52）°逐渐减小，至术后半年时基本稳定（0.97±1.13）°；末次随访VAS 评分由术前（5.75±1.32）分下降至（1.13±1.36）分，差异有统计学意义（t=13.44，P=0.00）；Lysh?觟lm评分由（50.13±11.57）分增至术后（91.97±3.04）分，手术前后对比差异有统计学意义（t=-20.20，P=0.00）；术后专科查体未显示关节失稳。结论：交叉韧带囊肿临床易于诊断，其发病可能由多种因素引起，关节镜下囊肿切除术能有效改善临床症状。
