摘要:
目的:探索血清基础促性腺激素水平及盆腔超声在中枢性性早熟(CPP)女童中的诊断价值。方法:79例因乳腺发育就诊的女童,年龄2.6~10.0岁,平均(7.5±1.5)岁。行促性腺激素释放激素(GnRH)激发试验分为CPP组和乳房早发育(PT)组,作ROC曲线来评估血清基础黄体生成素(LH)、促卵泡刺激素(FSH)、基础LH/FSH比、雌二醇(E2)水平和盆腔超声参数的敏感度和特异度并测量曲线下面积。结果:基础LH临界值为0.19 IU / L,基础LH / FSH比为0.07,子宫长径2.27 cm,卵巢容积1.15 mL,诊断CPP的敏感度分别为80%、85%、87.5%、92.5%,特异度分别为84.6%、76.9%、48.7%、30.8%。结论:血清基础LH,基础LH/FSH比,子宫长径和卵巢容积可能是CPP可靠的预测因子。基础LH和基础LH/FSH比的临界值有较高的特异度。
Abstract:
Objective: To explore the value of serum basal gonadotropin levels and pelvic ultrasound in the diagnosis of central precocious puberty(CPP)in girls. Methods:The gonadotropin-releasing hormone (GnRH) stimulation test was performedin the seventy-nine girls aged range from 2.6 to 10.0 years old,mean age:(7.5±1.5) years, who stayed in hospital for breastdevelopment.Theywere divided into CPP group and early breast development group (PT).Usingreceiver operating curves (ROCs), the sensitivity and specificity of serum basal luteinising hormone (LH), follicle stimulating hormone (FSH), basal LH/FSH ratio, oestradiol(E2)levels and pelvic ultrasonic parameters were evaluatedand theareaunder curve (AUC) was measured.Results: BasalLH cut-off values was 0.19 IU/L and a basal LH/FSH ratio was 0.07, the uterinelength was2.27 cm, the ovarian volumewas ??1.15 mL,the sensitivity were 80%,85%,87.5% and 92.5%, respectively. The specificity were84.6%,76.9%,48.7% and 30.8%, respectively.Conclusion: Serum basalLH, basal LH/FSH ratio, uterine length and ovarian volume maybe the predictors for CPP.Cut-off values of basal LH and the basal LH/FSH ratio have high specificity.
