目的:探讨低剂量吸入性糖皮质激素(ICS)对轻中度持续性哮喘患儿治疗期间及停药后6~12个月生长的影响。方法:将100例经过低剂量ICS治疗至少1年的轻中度持续性哮喘患儿设为哮喘组,年龄3~10岁,平均(6.8±1.7)岁。又根据吸入ICS治疗的时间是否>2年分为长期组(>2年,平均疗程为2.91年±0.51年)和短期组(≥1年且≤2年,平均疗程为1.91年±0.19年)。60例健康儿童作为对照组。回顾性观察不同疗程的哮喘患儿治疗期间及停药后6~12个月身高治疗年增长率(GV)、体质指数(BMI)及体质量年增长率(每年体质量增长的公斤值)的变化。结果:使用低剂量ICS的哮喘患儿其BMI、GV与正常对照组相比差异无显著性(P>0.05),而两组的体质量年增长率有统计学意义(P=0.037),哮喘组的体质增长率高于对照组;哮喘组患儿停药前及停药后GV及体质年增长率差异无显著性(P>0.05);长期组与短期组相比,其BMI、GV及体质增长率在两组间的差异均无显著性(P>0.05)。结论:低剂量ICS中长疗程的吸入治疗,对哮喘患儿治疗期间及停药后6~12个月的生长无明显影响。
Abstract:
Objective To explore the treatment of low dose inhaled glucocorticoid on the growth of children with mild-to-moderate persistent asthma.MethodsA retrospective study was conducted on 100 asthmatic children compared with 60 control children. Asthmatic children(aged 3 to 10 years) presented with mild-to-moderate asthmatook received therapywith low dose inhaled glucocorticoid for at least 1 year. The patients were divided into two groups based on the treatment duration:long-term group(>2 years)and short-term group(≥1 year and ≤2 years).The children’s annual growth rate (GV), body mass index (BMI) and annual weight growth rate (kg values of weight gain per year) were compared between the two groups. ResultsTheasthmatic children’s BMIandGV showed no significant difference from normal control group (P>0.05), while the annual weight growth rate of the two groups hadsignificant difference (P =0.037).The GV and annual weight growth rate had no significant differences before the withdrawal and after the withdrawal of the asthma children(P>0.05). The BMI and GV had no significant difference between the long term and short term groups (P>0.05).Conclusion:The use of low dose inhaled glucocorticoid in long time may have no effect on the growth of the asthma children.
