摘要:
目的:研究分化型甲状腺癌(DTC)患者131I治疗前肝功能损害与甲状腺功能减退的关系。方法:收集拟行131I治疗前的200例DTC患者的临床资料,并根据患者接受131I治疗前促甲状腺激素(TSH)值的不同,将其分为1组(TSH<30 mU/L)、2组(30 mU/L <TSH<100 mU/L)、3组(100 mU/L <TSH<150 mU/L)、4组(TSH>150 mU/L)。根据各组临床资料,使用SPSS17.0软件进行单因素方差分析、独立样本t检验、Pearson及Spearman相关分析其差异。结果:200例患者中肝功能正常组145例,肝功能异常组55例,其中肝功能异常组中丙氨酸转氨酶(ALT)异常者36例、天冬氨酸转氨酶(AST)异常者18例、γ谷氨酰转肽酶(γ-GGT)异常者18例、总胆红素(TBIL)及直接胆红素(DBIL)异常者8例。当TSH处于不同阶段时,相应的血清白蛋白、ALT、AST及碱性磷酸酶均值未见明显差异(p>0.05)。但当30 mU/L <TSH<100 mU/L,其相对应的γ-GGT、TBIL及DBIL均高于TSH>150 mU/L组(p<0.05)。性别、年龄及TSH值对于肝功能的影响有统计学意义(p <0.05)。是否伴有桥本甲状腺炎及是否合并局部或远处转移对肝功能无明显影响(p>0.05)。结论: DTC患者131I治疗前甲状腺功能减退对肝脏转氨酶无明显影响,主要引起肝脏γ谷氨酰转肽酶、总胆红素及直接胆红素的异常升高。
