目的:评价可吸收止血粉在手部骨折术中止血效果及术后创面愈合功能中的应用效果。方法:选取2024年5月—2024年12月河北医科大学第三医院手外科收治的92例手部开放性骨折患者为研究对象,采用随机数字表法将其分为研究组和对照组,各46例。对照组采用常规吸收性明胶海绵配合电凝止血,研究组在对照组基础上喷洒可吸收止血粉(主要成分为微孔多聚糖)止血。比较两组止血效果(5 min止血成功率、止血总时间及迟发性出血)、手术前后实验室指标、术后创面愈合情况,并进行安全性评价。结果:与对照组相比,研究组止血总时间缩短(t=4.479,P<0.001),围术期总失血量减少(t=13.317,P<0.001),迟发性出血率降低(P=0.035);两组5 min止血成功率无统计学差异(P>0.05)。术后1 d对照组凝血酶时间(TT)高于研究组,纤维蛋白原(FIB)低于研究组(t=4.340、3.403,均P<0.05),两组其余指标在术后1 d、1个月均无统计学差异(均P>0.05)。两组创面愈合等级比较差异无统计学意义(P>0.05)。两组均未出现急性血肿及止血相关并发症,安全性均较好。结论:微孔多聚糖可吸收止血粉在手部骨折术中止血效果满意,有助于缩短止血时间、降低迟发性出血风险,改善凝血指标,并且有助于创面愈合,安全性好。
Abstract:
Objective: To evaluate the hemostatic efficacy and postoperative wound-healing function of absorbable hemostatic powder in hand fracture surgery. Methods: A total of 92 patients with open hand fractures admitted in Department of Hand Surgery, the Third Hospital of Hebei Medical University from May 2023 to December 2024 were selected as subjects. Using a random number table method, they were divided into a study group and a control group, with 46 patients in each group. The control group received conventional absorbable gelatin sponge combined with electrocoagulation for hemostasis, while the study group was additionally treated with absorbable hemostatic powder (main component: microporous polysaccharide) for hemostasis. Hemostatic outcomes (5-minute success rate, total hemostasis time, and delayed bleeding), preoperative/postoperative laboratory indicators, postoperative wound healing were compared between the two groups, and safety evaluation was conducted. Results: Compared with the control group, the study group exhibited shorter total hemostasis time (t=4.479, P<0.001), reduced perioperative total blood loss (t=13.317, P<0.001), and lower delayed bleeding rate (P=0.035), while no statistical difference was observed in 5-minute hemostasis success rate (P>0.05). The thrombin time (TT) of the control group was higher than that of the study group, and the fibrinogen (FIB) was lower than that of the study group on one day after operative (t=4.340, 3.403, both P<0.05), with no differences in other indicators at one day or one month after operative (all P>0.05). No statistical difference was found in wound healing grades between the two groups (P>0.05). No acute hematomas or hemostasis-related complications occurred in either group, indicating good safety. Conclusion: Microporous polysaccharide absorbable hemostatic powder demonstrates satisfactory hemostatic efficacy in hand fracture surgery. It contributes to reducing hemostasis time, lowering delayed bleeding risk, improving coagulation indicators, and promoting wound healing, with favorable safety.
