摘要:
目的:了解医疗场所医务人员遭受攻击事件的发生频率及其分布特征,分析攻击事件发生的起因及应对措施。方法:采用多阶段分层抽样方法选取天津市4所医院的500名医务人员作为样本,通过问卷调查收集相关信息,采用描述性方法了解这些医务人员在1年内是否遇到患者及其家属攻击,如果遇到,说明具体情况。结果:本次共发放500份调查问卷,收回有效问卷475份,应答率95%。调查对象中有332人(69.89%)表示在工作场所遇到过攻击事件。按人口社会学特征对调查对象进行分组比较,发现30岁以下、本科及以下学历、工作年限小于5年、护理岗位人员遭受攻击事件的发生率较高(P<0.05),性别和医院级别因素对是否遭受攻击事件影响不大(?字2=0.97/1.69,P =0.528/0.430>0.05)。Logistic 回归分析发现工作年限超过10年、博士学历、行政及其他岗位的医务人员遭受更少的攻击事件(P<0.05)。攻击事件常表现为辱骂、威胁、推搡等语言及肢体暴力,主要原因是医患沟通低效、患者及家属对治疗效果不满。结论:医疗场所医务人员中,年轻医师和护士遭受攻击的发生率较高,需进一步完善在职培训制度,提高年轻医生和护士的业务水平和医患沟通能力,加强医学职业精神教育。
Abstract:
Objective: To understand the frequency of attacks and their distribution characteristics at the medical staff in hospital, and to analyze the causes and countermeasures of the attacks. Methods: We used multistage stratified sampling method to extract the 500 medical personnels in four hospitals of Tianjin, through filling in the questionnaire survey to collect the basic information. We also investigated the attack events within 1 year. We analyze the results of the surveys by using descriptive method. Results: A total of 500 questionnaires were issued, 475 of which were valid, with a response rate of 95%. At least 332 people (69.89%) suffered from workplace attacks in all of the 495 respondents. According to the different characteristics of demographic sociology for grouping investigation and comparison, we found that the incidences of attacks appeared to be higher in the group with the age below 30, at the bachelor degree, with the working duration of less than five years, and in the nursing jobs(P values are less than 0.05), but no difference was found in the groups of different gender or of the different hospital levels (chi-square value = 0.97/1.69, P = 0.528/0.430). Logistic regression analysis indicated that working duration of more than ten years, Ph.D. degree, and administrative positions suffered less attack incidents(P<0.05).Language and physical violence, such as abuse, threat, pushing and so on, are the main causes of the attack. The main reasons are poor communication between doctors and patients, and the dissatisfaction of patients and their families with the treatment effect. Conclusion: Among the medical staff in the medical places, the incidence of young doctors and nurses being attacked is high. It is necessary to further improve the on-the-job training system, improve the professional level of young doctors and nurses and the ability of doctor-patient communication, and strengthen the education of medical professionalism.
