摘要:

目的：探讨妊娠期糖尿病（GDM）患者血浆中肿瘤坏死因子-α（TNF-α）与白细胞介素-6（IL-6）的表达水平及意义。方法：选取2017年10月—2018年3月在我院诊治的妊娠期糖尿病孕妇130例作为GDM组，正常孕妇100例作为正常组，采用酶联免疫吸附试验（ELISA）测定所有研究对象血浆中TNF-α与IL-6的浓度，分析TNF-α和IL-6与胰岛素抵抗指数（HOMA-IR）的相关性。结果：GDM组血浆中TNF-α、IL-6、空腹血糖（FBG）、空腹胰岛素（FINS）、HOMA-IR的水平与正常组相比明显升高（Ｐ<0.05）。GDM组血浆中TNF-α与IL-6浓度呈正相关（Ｐ<0.05）；TNF-α、IL-6分别与FBG、FINS、HOMA-IR呈正相关（Ｐ<0.05）。正常组血浆中TNF-α与IL-6的浓度无相关性（Ｐ>0.05），TNF-α、IL-6分别与FBG、FINS、HOMA-IR无相关性（Ｐ>0.05）。结论：TNF-α和IL-6与GDM病情的严重程度相关，并可能通过胰岛素抵抗参与GDM的发病。

Abstract:

Objective: To investigate the expression and significance of tumor necrosis factor-α(TNF-α) and interleukin-6(IL-6) in plasma of patients with gestational diabetes mellitus(GDM). Methods: One hundred and thirty pregnant women diagnosed with gestational diabetes in our hospital from October 2017 to March 2018 were selected as the GDM group, and 100 normal pregnant women were chosen as the normal group. The concentrations of TNF-α and IL-6 in plasma of all subjects were determined by enzyme-linked immunosorbent assay (ELISA), and the correlations between TNF-α and IL-6 and insulin resistance index(HOMA-IR) were analyzed. Results: The levels of TNF-α, IL-6, fasting blood glucose(FBG), fasting serum lisulin(FINS) and HOMA-IR in the plasma of GDM group were significantly higher than those in the normal group(P<0.05). There was a positive correlation between TNF-α and IL-6 in plasma of GDM group (P<0.05). TNF-α and IL-6 were positively correlated with FBG, FINS and HOMA-IR(P<0.05). There was no correlation between the levels of TNF-α and IL-6 in the normal group(P>0.05). There were no correlations found between TNF-α and VCAM-1 and FBG, FINS and HOMA-IR(P>0.05). Conclusion: TNF-α and IL-6 are associated with the severity of GDM and may be involved in the pathogenesis of GDM through insulin resistance.
