摘要:

目的 :探讨胆管支架术及更换时间对内镜下十二指肠乳头括约肌切开术（EST）取石失败老年复杂性胆总管结石患者结石清除效果及并发症的影响。方法：选取EST术取石失败老年复杂性胆总管结石患者共150例，以随机数字表法分别分为A组和B组，每组各75例；其中A组患者术后每6个月更换胆管支架，而B组患者则术后每3个月更换胆管支架；比较两组患者术后随访结石清除率、结石直径、结石数量及术后并发症发生率。结果：两组患者术后随访结石清除率比较差异无统计学意义（P>0.05）；两组患者术后随访结石直径和结石数量比较差异无统计学意义（P>0.05）；B组患者术后并发症发生率显著低于A组，差异有统计学意义（P<0.05）。结论：EST术取石失败老年复杂性胆总管结石患者行胆管支架放置在清除结石方面效果确切，而缩短术后胆管支架更换时间有助于降低术后并发症发生风险，提高治疗安全性。

Abstract:

bjective: To investigate the influence of replacement time of biliary stent on stone removal efficiency and complications of elderly patients with complicated common bile duct stones and stone removal failure by EST. Methods: One hundred and fifty elderly patients with complicated common bile duct stones and stone removal failure by EST were randomly divided into two group including A group (75 patients) with postoperative biliary stent replacement for every 6 months and B group (75 patients) with postoperative biliary stent replacement for every 3 months. And the stone removal efficiency, the stone diameter and the stones number with follow-up after operation and the incidence of postoperative complications of both groups were compared. Results: There was no significant difference in the stone removal efficiency during follow-up after operation between 2 groups（P>0.05）. There was no significant difference in the stone diameter and the stones number during follow-up after operation between 2 groups（P>0.05）. The incidence of postoperative complications of B group were significantly lower than A group（P<0.05）. Conclusion: Biliary stent in the treatment of elderly patients with complicated common bile duct stones and stone removal failure by EST may have the reliable effects on stone removal; and shortening the replacement time of biliary stent can efficiently reduce the risk of postoperative complications and higher the treatment safety.
