摘要:
目的:探讨磁共振弥散张量成像(DTI)联合听性脑干反应对孤独症谱系障碍(ASD)的诊断价值。方法:选取2019年1月—2022年1月就诊的ASD患儿60例(观察组),其中轻中度患儿40例,重度患儿20例,同时选取正常儿童60名作为对照组,分析DTI参数各向异性分数(FA)、表观扩散系数(ADC)及脑干听觉诱发电位(BAEP)参数差异。结果:观察组胼胝体压部FA明显高于对照组(t=6.433,P<0.05),而ADC明显低于对照组(t=8.468,P<0.05)。观察组Ⅰ波潜伏期较对照组缩短(t=3.417,P<0.05)。观察组重度患儿胼胝体压部FA明显高于轻中度患儿(t=2.457,P<0.05),而胼胝体压部ADC和Ⅰ波潜伏期明显低于轻中度患儿(t=4.564,P<0.001;t=3.519,P<0.05)。胼胝体压部FA、胼胝体压部ADC和Ⅰ波潜伏期预测重度ASD的受试者工作特征(ROC)曲线下面积分别为0.667、0.789和0.700(均P<0.05)。结论:核磁定量参数联合听性脑干反应在ASD的诊断和患儿疾病严重程度鉴别中具有一定的价值。
Abstract:
Objective:To investigate the diagnostic value of magnetic diffusion tensor imaging(DTI)combined with auditory brainstem response in autism spectrum disorder(ASD). Methods:A total of 60 children with ASD in hospital from January 2019 to January 2022 were selected as the observation group,among them,40 cases were mild to moderate,20 cases were severe,and 60 normal children were selected as the control group. The differences of fractional anisotropy(FA),apparent diffusion coefficient(ADC)and brainstem auditory evoked potential(BAEP)were analyzed. Results:The FA of the corpus callosum in the observation group was significantly higher than that in the control group(t=6.433,P<0.05),while the ADC was significantly lower than that in the control group(t=8.468,P<0.05). The latency of waveⅠin the observation group was significantly shorter than that in the control group(t=3.417,P<0.05). The FA of the splenium of corpus callosum in severe children in the observation group was significantly higher than that in mild to moderate children(t=2.457, P<0.05). The ADC and waveⅠlatency of the splenium of the corpus callosum were significantly lower than those of mild to moderate children(t=4 . 564 ,P<0 . 001 ;t=3 . 519 ,P<0 . 05). The area under the ROC curve of the FA of the corpus callosum,the ADC of the corpus callosum and the latency of the first wave in predicting severe ASD were 0.667,0.789 and 0.700,respectively(all P<0.05). Conclusion:MRI quantitative parameters combined with auditory brainstem response have certain value in the diagnosis of ASD and the differentiation of disease severity in children.
