摘要:
 目的:研究代谢综合征和非杓型血压与老年高血压患者右心室结构及功能改变之间的关系。方法:采用横断面调查研究方法对442例老年高血压患者进行研究,根据病例特点分为代谢综合征组(n=242)和非代谢综合征组(n=200),完善24 h动态血压监测及超声心动图检查。比较两组间患者非杓型血压、杓型血压与超声心动图右心室舒张早期峰值速度/舒张晚期峰值速度(Em/Am)和心肌做功指数(Tei指数)关系。结果:伴有代谢综合征的老年高血压患者较无代谢综合征的患者24 h平均血压及夜间平均血压均较高(P<0.05)。伴有代谢综合征的非杓型血压患者右心房舒张长径、横径及Tei指数均升高(P<0.05),而右心室Em/Am较杓型血压组降低(P<0.05)。代谢综合征及血压昼夜节律在右心室重构中存在交互作用(P<0.05)。结论:代谢综合征、非杓型血压及二者相互作用可促进老年高血压患者右心室重构。
Abstract:
Objective:To investigate the role of the metabolic syndrome and non-dipper blood pressure in the change of right ventricular structure and function in senior hypertension patient. Methods: Four hundred and forty two senior hypertension patients were recruited. Based on having the metabolic syndrome or not, they were divide into metabolic syndrome group (n=242) and non-metabolic syndrome group (n=200). 24 h blood pressure monitoring and the ultrasonic cardiogram examination scales of the patients were collected. The dipper, non-dipper and right ventricular Em/Am and Tei index were compared between the two groups. ResultsCompared to the senior hypertension patients without the metabolic syndrome, the metabolic syndrome group had higher 24h mean pressure and the night mean pressure(P<0.05). In the metabolic syndrome group, the non-dipper sub-group had a higher right ventricular diastolic diameter, right atrium diastolic diameter and Tei index than the dipper sub-group (P<0.05). And the right ventricular Em/Am was decreased in the non-dipper sub-group as compared to the dipper one (P<0.05). ConclusionThe metabolic syndrome, non-dipper blood pressure and their interaction can improve the right ventricular remodeling in senior patients.
