目的:探讨肺康复联合岩盐气溶胶疗法对稳定期尘肺病患者肺功能、炎症水平及生活质量的疗效。方法:选择2025年6月至10月于天津医科大学第二医院康复医学科治疗的90例患者为研究对象,按照随机数字表法随机分为对照组(单纯肺康复,n=45)与试验组(肺康复+岩盐气溶胶治疗,n=45),干预8周。于干预前、后检测心肺功能指标每千克体重峰值摄氧量(pVO2 /kg)、用力肺活量(FVC)、第一秒用力呼气容积占预计值百分比(FEV1%)、最大通气量(MVV)、血清炎症因子白细胞介素-8(IL-8)、γ干扰素水平(IFN-γ),并在随访6个月时评估患者症状与生活质量水平。结果:治疗后,两组患者心肺功能指标均有所提升,试验组pVO2 /kg、FVC、FEV1%及MVV改善优于对照组(t=-2.01、-2.96、-5.49、-2.67,均P <0.05)。试验组IL-8水平降低,IFN-γ水平升高(t=3.33、-4.17,均P <0.01),对照组炎症指标变化不显著。6个月时试验组mMRC分级、SGRQ评分低于对照组(z=-3.56,t=9.37,P<0.05)。试验组临床总有效率为91.1%,高于对照组的73.3%(χ2=4.87,P=0.03)。结论:肺康复联合岩盐气溶胶治疗可更有效地改善尘肺病患者的心肺功能、减轻系统性炎症并提升生活质量,其疗效优于单纯肺康复。
Abstract:
Objective: To investigate the efficacy of pulmonary rehabilitation(PR) combined with rock salt aerosol therapy(RSAT) on lung function, inflammation level, and quality of life in stable pneumoconiosis patients. Methods: Ninety patients treated in the Department of Rehabilitation Medicine at the Second Hospital of Tianjin Medical University from June to October 2025 were selected as the study subjects and randomly divided into a control group (PR alone, n=45) and an experimental group (PR+RSAT, n=45) according to a random number table, with an intervention period of 8 weeks. Cardiopulmonary function indicators including peak oxygen uptake per kilogram of body weight (pVO2 /kg), forced vital capacity (FVC), forced expiratory volume in one second as a percentage of the predicted value (FEV1%), maximal voluntary ventilation (MVV), serum inflammatory cytokine interleukin-8 (IL-8)and interferon-gamma (IFN-γ) were measured pre- and post-intervention. Patients′ symptoms and quality of life were assessed at a 6-month follow-up. Results: Both groups showed improved cardiopulmonary function indicators, with the experimental group showing greater improvements in pVO2/kg, FVC, FEV1%, and MVV compared to the control group (t=-2.01, -2.96, -5.49, -2.67, all P<0.05). The experimental group exhibited a significant decrease in IL-8 and increase in IFN-γ (t=3.33, -4.17, both P<0.01), while the control group showed no significant changes in inflammatory indicators. At 6 months, the mMRC grading and the SGRQ scores of the experimental group was significantly lower than that of the control group (z=-3.56, 9.37, both P=0.01). The total clinical effective rate of the experimental group was 91.1%, which was higher than the 73.3% of the control group (χ2=4.87, P=0.03). Conclusion: PR combined with RSAT is more effective than PR alone in improving cardiopulmonary function, modulating systemic inflammation, and enhancing quality of life in patients with stable pneumoconiosis.
